Application, or just complete it on-line, print it out and fax it to us.
After completing the Application, be sure you have signed the signature page. For merchants that process less than $500,000.00 a year on Visa and MasterCard they will need to sign in the Personal Guarnatee section. For processing over $500,000.00 the merchant must sign in the middle of the document as a corporate officer and supply a copy of the Corporation's most recent Federal Tax Return.
